Dental Patient Records

Help us to help you by keeping us informed with your current address and contact details.

When visiting the practice we may ask you to fill in a medical history questionnaire, this is to assist in our diagnosis and treatment. This will also include details of any medications and drugs that you are taking, so please bring these details with you.

All patient information is treated with the strictest confidence. Your dental records will remain confidential and secure. However from time to time we may need to release these to NHS England or the NHS Business Services Authority as part of our ongoing clinical review process. We will contact you before releasing your dental records to another organisation.

Alternatively for any emergency conditions you may attend the A&E department or Contact NHS 111 from any landline
